OSHA is investigating a fatal accident Monday afternoon at Crown Equipment’s Plan 4 operations in New Bremen, Auglaize County.
The worker, 49-year-old forklift operator Travis Temple, was struck by a lift truck about 2:15 p.m. and died later at St. Rita’s Medical Center in Lima, according to a statement from Randy Niekamp, vice president of human resources. Temple had been with Crown for more than seven years, Niekamp said.
The company notified the Occupational Safety and Health Administration and is cooperating with the accident investigation to determine its cause and no additional information about the incident will be available until the agency’s investigation is complete, Niekamp said in the statement.
“We are deeply saddened by this tragic event and wish to extend our sincere condolences to the family and friends of Mr. Temple,” Niekamp said in the statement.
